Admission App Form 2012ISB Student Application Form needs to be completed and submitted together with :

• Photocopies of passports (cover page, picture page, vital information page, expiration date, and current visa page) of student and both parents; the original must be present at the time of submission of the application or upon arrival prior to admissions.

• A photocopy of the KITAS (Limited Stay Permit) for student and both parents; the original must be present for verification. If the KITAS is in process, a letter from the sponsoring company (employer) is required stating this and then the original KITAS must be present within 30 days of enrolment. Diplomatic and Dinas visa holders must provide photocopies of the passport and the current visa pages.

• Two (2) photographs of the applicant (4X6 cm). Color photos should have a red background (passport-size). APPLICATION FORM

Application forms should be completed after a visit has been arranged. Hard copies of the application forms are available at the school or you can print the attached copy. Please note that online applications will be available from May 2011.
